Office Policies

  • Payment in full for amounts not covered by insurance are required at time of service for all consultations/office visits. No exceptions.
  • Bring current medicines, insurance card(s) and drivers license to all visits.
  • Cancellations within less than one business day and or "no shows" in the office are subject to a $50 charge.
  • Cancellations of procedures within less than two business days are subject to a $150 charge.
